🧠 왜 중요한가요

억제대는 덜 제한적인 방법이 실패한 뒤의 최후의 수단이에요: 재지남력 제공, 보호자(시터), 침대 알람, 환자를 간호사실 가까이로 옮기기, 통증이나 초조의 원인 치료가 해당돼요. 의료진의 처방이 필요하고, 응급 상황에서는 간호사가 억제대를 적용하고 직후에 처방을 받아야 하며, 폭력적이거나 자해적인 행동에 대한 억제대는 한 시간 이내에 대면 평가도 필요해요. PRN 억제대 처방은 절대 허용되지 않아요. 폭력적이거나 자해적인 행동에 대한 처방은 성인은 4시간마다, 9세에서 17세는 2시간마다, 9세 미만 아동은 한 시간마다 갱신해야 하고, 비폭력 의학적 억제대 처방은 병원 정책에 따라 갱신하며, 보통 달력상 하루마다예요. 억제대는 빠른 해제 매듭으로 침대 난간이 아닌 침대 틀에 묶고, 손가락 두 개가 들어갈 여유를 둬요. 순환, 피부, 감각을 자주 확인하고, 보통 정책에 따라 15분에서 30분마다 하고, 최소 2시간마다 풀고 체위를 바꿔 주며, 수분과 배뇨를 제공하고, 행동, 시도한 대안, 환자의 반응, 각 점검을 기록해요. 행동이 가라앉으면 바로 억제대를 제거해요.

Restraints are a LAST RESORT after less restrictive measures fail: reorientation, a sitter, a bed alarm, moving the client near the station, treating pain or the cause of agitation. A provider's ORDER is required; in an emergency the nurse may apply restraints and must obtain the order immediately afterward, and restraint for violent or self-destructive behavior also needs a face-to-face evaluation within one hour. PRN restraint orders are never allowed. Orders for violent or self-destructive behavior must be renewed every 4 hours for adults, every 2 hours for ages 9 to 17 and every hour for children under 9; orders for non-violent medical restraints are renewed as hospital policy sets, commonly every calendar day. Tie restraints with a QUICK-RELEASE knot to the BED FRAME, never the side rails, with two fingers of slack. Check circulation, skin and sensation often, commonly every 15 to 30 minutes per policy; release and reposition at least every 2 hours, offer fluids and toileting, and document the behavior, alternatives tried, the client's response and each check. Remove the restraint as soon as the behavior resolves.

🛏️ 혼란스러운 환자가 IV를 잡아당겨요. 간호사는 소매로 부위를 덮고, IV 펌프를 시야 밖으로 옮기고, 가족에게 전화해 곁에 앉아 달라고 하고, 진정 약물을 검토해 달라고 의료진에게 요청해요. 그 어느 것도 억제대가 아니며, 장갑형이나 손목 억제대를 논의하기 전에 이 모든 단계를 시도하고 기록해야 해요.

🛏️ A confused client pulls at his IV. The nurse tries covering the site with a sleeve, moves the IV pump out of view, calls a family member to sit, and asks the provider to review the sedating medication. None of that is a restraint, and every one of those steps must be tried and documented before a mitt or wrist restraint is even discussed.
