Don't Argue With the Delusion, and Other Rules for Psychosis
👂 A client says the nurses are poisoning the food. Telling him that is ridiculous makes you one of the poisoners. Agreeing makes it true. There is a third path, and it is the only one that passes.
💡 Idea clave
Acknowledge the feeling, not the content. Never argue with a delusion, always ask what the voices say, and treat command hallucinations as an emergency.
For HALLUCINATIONS, acknowledge the experience without confirming it: 'I don't hear the voices, but I can see they're frightening you.' Ask what the voices are saying; COMMAND hallucinations telling the client to harm self or others are a safety emergency. Focus on reality and present activities. For DELUSIONS, do not argue, do not agree, and do not try to logic them away; address the underlying feeling ('You feel unsafe here') and redirect. Avoid touching a paranoid client without warning, do not whisper near them, and offer packaged foods if they fear poisoning. Keep communication simple and concrete; avoid humor and abstract phrases with a client who thinks concretely. Set clear, consistent LIMITS for aggressive behavior and maintain a safe distance and an exit path. POSITIVE symptoms (hallucinations, delusions, disorganized speech) respond better to medication than NEGATIVE symptoms (flat affect, avolition, social withdrawal). Common DEFENSE MECHANISMS the exam names: DENIAL (refusing to accept reality), PROJECTION (attributing one's own feelings to others), DISPLACEMENT (redirecting feelings to a safer target), RATIONALIZATION (excusing with logical reasons), REGRESSION (returning to earlier behavior), SUBLIMATION (channeling into acceptable activity), REACTION FORMATION (acting the opposite of what one feels), and COMPENSATION.
🍱 The client who fears poisoning is offered sealed yogurt and a packaged sandwich, which he eats. The nurse says, 'It must be exhausting to feel you can't trust anyone here.' He talks for ten minutes. Nothing about the delusion was debated, and the client ate, rested and engaged. The exam's correct answer usually looks this undramatic.
I don't hear it, but I believe you do'; ask what the voices say
- Ideas delirantes: atiende el sentimiento, redirige, nunca discutas ni des la razónDelusions: address the feeling, redirect, never argue or agree
✅ Ponte a prueba
A client says, 'The voices are telling me to hurt my roommate.' What is the priority nursing action?
- Animar al paciente a ignorar las vocesEncourage the client to ignore the voices
- Documentar y volver a valorar en una horaDocument and reassess in one hour
- Decirle al paciente que las voces no son reales y que pasaránTell the client the voices are not real and will pass
- Quedarse con el paciente y mantener a salvo al compañeroStay with the client and keep the roommate safe
Command hallucinations to harm others are a safety emergency requiring immediate protective action: stay with the client, remove the roommate from danger and notify the team.
A client insists the FBI has planted a device in her tooth. Which nurse response is most appropriate?
- '¿Por qué le interesaría usted al FBI?''Why would the FBI be interested in you?'
- 'No hay ningún dispositivo; eso es imposible.''There is no device; that's impossible.'
- 'Es cierto que el FBI vigila a algunas personas.''The FBI does monitor some people.'
- 'Eso suena aterrador. ¿Qué la hace sentirse insegura ahora mismo?''That sounds terrifying. What makes you feel unsafe right now?'
Respuesta: D · 'Eso suena aterrador.
Acknowledge the emotion and redirect without arguing or validating the delusional content.
A client who is angry at his provider yells at the nursing assistant instead. Which defense mechanism is this?
- ProyecciónProjection
- NegaciónDenial
- SublimaciónSublimation
- DesplazamientoDisplacement
Respuesta: D · Desplazamiento
Displacement redirects feelings toward a less threatening target. Projection would attribute the anger to the assistant.
