¿PN o RN? La misma máquina, distinto alcance

PN or RN? Same Machine, Different Scope

⚖️ Two candidates, one Pearson VUE room: one takes the NCLEX-PN, the other the NCLEX-RN. Same five hours, same 85 to 150 items, same case studies. What differs is the job the questions assume you have. LPN/LVN student using this app? This is your translation key.

💡 Idea clave

Same test engine, same clinical judgment steps, same safety rules. On the PN exam the nurse in the stem is an LPN/LVN, so filter every option through what a practical nurse collects, reinforces, monitors and reports.

Both exams are COMPUTER ADAPTIVE, run 85 to 150 items in five hours, and have used the Clinical Judgment Measurement Model since the Next Generation launch in April 2023: at minimum length, 52 content items, three six-item case studies and 15 unscored pretest items, with partial credit on multi-part items. Each exam has its OWN test plan and its own passing standard, set by NCSBN. The 2026 NCLEX-PN test plan (effective April 2026) has eight categories: COORDINATED CARE 18 to 24% (the RN plan's Management of Care, 15 to 21%), SAFETY AND INFECTION PREVENTION AND CONTROL 10 to 16%, HEALTH PROMOTION AND MAINTENANCE 6 to 12%, PSYCHOSOCIAL INTEGRITY 9 to 15%, BASIC CARE AND COMFORT 7 to 13%, PHARMACOLOGICAL THERAPIES 10 to 16% (the RN plan's Pharmacological and Parenteral Therapies, 13 to 19%), REDUCTION OF RISK POTENTIAL 9 to 15% and PHYSIOLOGICAL ADAPTATION 7 to 13%. The real difference is SCOPE. In the NCLEX world the practical nurse COLLECTS DATA rather than performing the initial assessment, CONTRIBUTES TO the plan of care rather than writing it, REINFORCES teaching the RN already gave, MONITORS clients, including those receiving IV therapy and blood products, and REPORTS changes to the RN or the provider. IV items are framed around the site, the rate, spotting a complication, stopping what harms the client and reporting. For a PN student, this app's priority, delegation, safety, pharmacology, dosage, lab and Next Gen lessons apply as written; in the Management of Care lessons, read 'write the plan' as 'contribute to it' and 'teach' as 'reinforce'. The NCLEX-PN is used for US licensure only.

🔁 Dana, an LVN student, reads: 'Two hours after surgery a client's blood pressure is 86/50, down from 124/78, and the dressing is saturated. What should the nurse do first?' Same facts: falling pressure plus bleeding is hemorrhage. Same action: pressure on the site, reinforce the dressing, stay with the client. Only the escalation changes: an RN notifies the surgeon; Dana reports to the RN, who does. Same distractor on both exams: 'document and recheck in 15 minutes.'

📌 Para recordar

  • El mismo CAT, 85 a 150 ítems, cinco horas y seis pasos de juicio clínico; planes de examen y estándares de aprobación separadosSame CAT, 85 to 150 items, five hours and six clinical judgment steps; separate test plans and passing standards
  • Plan PN: Cuidado coordinado 18–24% (la más grande) y Terapias farmacológicas 10–16% reemplazan dos nombres de categorías del RNPN plan: Coordinated Care 18–24% (the largest) and Pharmacological Therapies 10–16% replace two RN category names
  • Alcance PN: recopilar datos, contribuir al plan, reforzar la enseñanza, monitorear, informar; la valoración y los planes de cuidados siguen siendo del RNPN scope: collect data, contribute to the plan, reinforce teaching, monitor, report; assessment and care plans stay RN

✅ Ponte a prueba

On the 2026 NCLEX-PN test plan, which category carries the largest share of items?
  1. Integridad psicosocial, 9 a 15%Psychosocial Integrity, 9 to 15%
  2. Cuidado coordinado, 18 a 24%Coordinated Care, 18 to 24%
  3. Terapias farmacológicas, 10 a 16%Pharmacological Therapies, 10 to 16%
  4. Adaptación fisiológica, 7 a 13%Physiological Adaptation, 7 to 13%

Respuesta: B · Cuidado coordinado, 18 a 24%

Coordinated Care, the PN counterpart of the RN plan's Management of Care, is the largest PN category at 18 to 24%. On the RN plan Management of Care is 15 to 21% and Pharmacological and Parenteral Therapies 13 to 19%.

¿Qué opción corresponde al alcance PN?A PN candidate reads: 'A client receiving a unit of packed red blood cells reports chills and low back pain. What should the nurse do first?' Which option fits PN scope?
  1. Disminuir la velocidad y hacer una valoración completa de pies a cabezaSlow the rate and perform a complete head-to-toe assessment
  2. Detener la transfusión, mantener la vía IV permeable con solución salina normal e informar al RNStop the transfusion, keep the IV open with normal saline and report to the RN
  3. Redactar un nuevo plan de cuidados por sospecha de reacción transfusionalWrite a new plan of care for a suspected transfusion reaction
  4. Continuar la unidad y enseñar al paciente sobre las reacciones transfusionalesContinue the unit and teach the client about transfusion reactions

Chills and back pain mid-transfusion suggest a reaction. Stopping the blood and keeping the line open are immediate PN-scope safety actions; then notify the RN. Assessment, care plans and initial teaching are RN work.

Which statement about the NCLEX-PN compared with the NCLEX-RN is accurate?
  1. El examen PN no tiene casos clínicos ni da crédito parcial en ningún ítemThe PN exam has no case studies and gives no partial credit on any item
  2. Ambos usan el mismo rango de 85 a 150 ítems y el límite de cinco horasBoth use the same 85 to 150 item range and five-hour limit
  3. El examen PN se califica por el porcentaje de ítems respondidos correctamenteThe PN exam is scored by the percentage of items answered correctly

Both run 85 to 150 items in five hours, adapt to the candidate, include three six-item case studies and are decided against a passing standard, not a percentage. What differs is the test plan and the assumed scope.
