The Question Is Hiding in the Last Sentence

🔍 Most wrong answers on the NCLEX are right answers to a question that was never asked. The stem tells you exactly what it wants. You just have to read the last sentence like a lawyer.

💡 Idea clave

Rephrase the stem in your own words before reading the options. Decide what kind of answer it wants: a finding, an action, a correct statement or an incorrect one.

Find the KEYWORDS. 'First', 'initial', 'priority', 'best', 'most important' and 'immediately' mean every option may be correct and you must rank them. 'Further teaching is needed' or 'needs clarification' flips the question: you are hunting for the WRONG statement. 'Indicates understanding' wants the correct statement. 'Expected finding' wants normal; 'needs follow-up' wants abnormal. Read for the client's age, the time frame (two hours post-op is not two days post-op) and the setting. Then rephrase the question in your own words before you look at the options, so the options cannot reframe it for you.

🧾 Stem: 'A client prescribed warfarin says the following. Which statement indicates a need for further teaching?' One option says 'I will eat the same amount of leafy greens each week.' That is correct teaching, so it is NOT the answer. The option 'I will take aspirin for my headaches' is the wrong statement, so it IS the answer. Candidates who skim 'teaching' and pick the sensible statement lose the point on a topic they know.

📌 Para recordar

  • Palabras clave como first (primero), priority (prioridad) y best (mejor) significan que debes ordenar las opcionesKeywords like first, priority and best mean rank the options
  • Reformula tú la pregunta antes de que las opciones puedan replantearlaRephrase the question yourself before the options can reframe it

✅ Ponte a prueba

Una pregunta pide cuál afirmación del paciente 'requires follow-up by the nurse' (requiere seguimiento del personal de enfermería). ¿Qué estás buscando?A question asks which client statement 'requires follow-up by the nurse.' What are you looking for?
  1. La afirmación más largaThe longest statement
  2. La afirmación que menciona al proveedor de saludThe statement that mentions the provider
  3. La afirmación que muestra un malentendido o un plan inseguroThe statement that shows a misunderstanding or unsafe plan

'Requires follow-up', 'needs clarification' and 'further teaching needed' all point to the incorrect or unsafe statement.

The stem says: 'Which action should the nurse take FIRST?' What does the word 'first' tell you about the options?
  1. La respuesta siempre es documentar primero la situaciónThe answer is always to document the situation first
  2. Varias opciones pueden ser adecuadas; ordénalasSeveral options may be appropriate; rank them

Priority words mean multiple options are plausible. The task is ordering, using frameworks like ABCs, Maslow and the nursing process.

Why rephrase the question in your own words before reading the options?
  1. Lo exige el software del examenIt is required by the testing software

Distractors are written to be attractive. Fixing what the question wants before you read them keeps you anchored.
