🫒 Pyloric stenosis feels like an olive. Intussusception looks like currant jelly. Hirschsprung's disease produces ribbons. The exam describes the sign and expects the diagnosis, in a 4-year-old's vocabulary.
💡 Pangunahing ideya
Olive mass with projectile vomiting: pyloric stenosis. Currant jelly stools and drawn-up knees: intussusception. Ribbon stools and no meconium: Hirschsprung. Sudden pain relief in appendicitis means rupture.
🧠 Bakit mahalaga
PYLORIC STENOSIS: PROJECTILE non-bilious vomiting after feeds in an infant 2 to 8 weeks old who is hungry again immediately, with an OLIVE-SHAPED mass in the right upper abdomen, dehydration and metabolic alkalosis from lost stomach acid; treated with surgery after fluid and electrolyte correction. INTUSSUSCEPTION: a telescoping bowel in a 3 month to 3 year old with sudden episodic severe abdominal pain, DRAWING THE KNEES UP, a sausage-shaped mass and CURRANT JELLY stools (blood and mucus); treated with an air or contrast enema, with surgery if that fails; a normal brown stool afterward suggests the reduction worked. HIRSCHSPRUNG DISEASE: missing nerve cells in the colon, so no meconium in the first 24 to 48 hours, chronic constipation, RIBBON-LIKE stools and a distended abdomen; treated with surgery to remove the affected bowel, often a single-stage pull-through (sometimes a temporary ostomy first). APPENDICITIS in children: periumbilical pain moving to the right lower quadrant, rebound tenderness, fever and vomiting; sudden relief of pain suggests PERFORATION; no heat, enemas or laxatives. COMMUNICABLE DISEASES: VARICELLA is contagious from 1 to 2 days before the rash until all lesions crust, and needs airborne plus contact precautions; MEASLES has Koplik spots and a rash spreading from the head down, with airborne precautions; FIFTH DISEASE shows a slapped-cheek rash and is a risk to pregnant contacts; SCARLET FEVER accompanies a group A strep infection, usually strep throat, with a sandpaper rash and a strawberry tongue; HAND-FOOT-AND-MOUTH shows oral and palm or sole lesions. Never give ASPIRIN to children with viral illness because of Reye's syndrome.
🎯 A 9-month-old screams and pulls up his legs every 20 minutes, then goes limp and pale between episodes. The diaper holds a dark red, jelly-like stool. The nurse recognizes intussusception, keeps the infant NPO and prepares for a contrast enema. An hour later a normal stool means the bowel has telescoped back. Watching the diaper was the evaluation step.

- I-document ang pagbuti at maghanda para sa dischargeDocument the improvement and prepare for discharge
Sudden relief of pain may indicate perforation: rupture relieves the pressure temporarily but leads to peritonitis, a surgical emergency, so the child stays NPO.
