Assess Before You Act (Unless the Stem Already Did)
📋 परीक्षा का सबसे लुभावना ग़लत जवाब वह परफ़ेक्ट intervention है जो एक कदम पहले दे दिया गया। Nursing process का एक क्रम है। परीक्षा जाँचती है कि आप उसका सम्मान करते हैं या नहीं।
📋 The most seductive wrong answer on the exam is the perfect intervention, delivered one step too early. The nursing process has an order. The exam checks whether you respect it.

अस्पष्ट stem हो तो पहले assess करें। जिस stem में data एक ही तरफ़ इशारा करे, वहाँ कार्रवाई करें। ऐसा assessment कभी न चुनें जो साफ़ तौर पर ज़रूरी emergency कार्रवाई में देरी करे।
Vague stem, assess first. Stem full of data pointing one way, act. Never choose an assessment that delays an obviously needed emergency action.
🧠 यह क्यों ज़रूरी है
Nursing process है ASSESSMENT, DIAGNOSIS, PLANNING, IMPLEMENTATION, EVALUATION। जब stem कोई अस्पष्ट या अधूरी स्थिति बताकर पहला कदम पूछता है, तो जवाब आमतौर पर ASSESSMENT होता है: मरीज़ को देखें, vital signs लें, auscultate करें, सवाल पूछें। जब stem पहले से assessment data दे चुका हो और तस्वीर साफ़ हो, तो जवाब INTERVENTION होता है। कौशल यह पहचानने में है कि आप किस स्थिति में हैं। जिस stem में लिखा हो ‘मरीज़ चक्कर आने की शिकायत करता है’ वह assessment माँगता है। जिस stem में लिखा हो ‘मरीज़ का blood pressure 78/40 है और वह पसीने से तर है’ उसमें assessment हो चुका है; वह कार्रवाई माँगता है। और अगर एकमात्र assessment विकल्प बेकार या ख़तरनाक हो (मरीज़ के दम घुटते समय chart देखना), तो उसे छोड़ दें।
The nursing process is ASSESSMENT, DIAGNOSIS, PLANNING, IMPLEMENTATION, EVALUATION. When a stem describes a vague or incomplete situation and asks for the first action, the answer is usually an ASSESSMENT: check the client, take vital signs, auscultate, ask a question. When the stem already gives you the assessment data and the picture is clear, the answer is an INTERVENTION. The skill is noticing which situation you are in. A stem that says 'the client reports feeling dizzy' wants an assessment. A stem that says 'the client's blood pressure is 78/40 and he is diaphoretic' has already assessed; it wants action. And if the only assessment option is pointless or dangerous (checking a chart while a client chokes), skip it.

🌡️ A client with pneumonia says, 'I feel hot.' Option A: give the PRN acetaminophen. Option B: take the temperature and check for chills. 'Feels hot' could be a warm room, anxiety, or a rising fever. The exam expects you to look before you act, so B is first. Change the stem to 'the temperature is 39.1 °C and acetaminophen is prescribed for a temperature over 38.5 °C'; now giving it is correct, because the assessment is done.
