May Mga Tell ang mga Distractor

Distractors Have Tells

🃏 Good distractors look like good nursing. But they have tells, the same way a bluffing poker player does: absolute words, passing the buck, and leaving the bedside. Spot the tell and you are down to two options.

💡 Pangunahing ideya

Absolutes, passing the buck, leaving the bedside and non-therapeutic responses are the common tells of a distractor. Narrow with them, then choose with content.

🧠 Bakit mahalaga

Unang tell: ABSOLUTES. Ang 'Always', 'never', 'all', 'none' ay bihirang totoo sa nursing, kaya pagdudahan ang mga ito. Ikalawang tell: PASSING THE BUCK. Ikatlong tell: LEAVING THE BEDSIDE.

Tell one: ABSOLUTES. 'Always', 'never', 'all', 'none' are rarely true in nursing, so treat them with suspicion. Tell two: PASSING THE BUCK. 'Notify the provider' or 'call the rapid response team' is usually not first when a nursing assessment or an immediate independent action (position, oxygen, stop the infusion) is available. The exception is a true emergency where the stem already gives you the assessment. Tell three: LEAVING THE BEDSIDE. The answer that keeps the nurse with an unstable client beats the one that sends you to the chart or the phone. Tell four: NON-THERAPEUTIC COMMUNICATION. Options that ask 'why', give false reassurance, or change the subject are almost never correct. These are heuristics, not laws. Use them to narrow, then decide with knowledge.

🚨 Stem: a client's chest tube drainage system is knocked over and the tubing disconnects from the chest tube. Options: notify the provider, document the event, place the end of the tube in a container of sterile water, or clamp the tube. Two options leave the problem unsolved while you talk or type. Clamping risks a tension pneumothorax. The immediate, independent nursing action is the sterile-water seal. Then you call. Then you chart.

📌 Tandaan

  • Manatili sa tabi ng unstable na pasyente; ang charting at pagtawag ay susunodStay with the unstable client; charting and phone calls come after

Which option is most likely a distractor based on wording alone?
  1. Mag-assess ng lung sounds ng pasyenteAssess the client's lung sounds
  2. I-position ang pasyente sa high Fowler'sPosition the client in high Fowler's
  3. Timbangin ang pasyente tuwing umagaWeigh the client each morning

'Never' is an absolute. Heart failure management restricts fluids; it does not eliminate them.

A client on a heparin infusion begins bleeding from the gums and IV site. The stem asks for the first action. Which option is best?
  1. I-notify ang providerNotify the provider
  2. Ihinto ang heparin infusionStop the heparin infusion
  3. I-document ang pagdurugoDocument the bleeding
  4. I-reassure ang pasyente na inaasahan itoReassure the client that this is expected

Stopping the infusion is an immediate, independent nursing action that addresses the cause. Then notify the provider and document.

Sabi ng isang nagdadalamhating pasyente, 'I don't know how I'll go on.' A grieving client says, 'I don't know how I'll go on.' Which nurse response is LEAST likely to be correct on the exam?
  1. Sasabihin ng nurse: 'It sounds like this feels overwhelming right now.''It sounds like this feels overwhelming right now.'
  2. 'Why do you feel that way?'
  3. Tahimik na umupo sa tabi ng pasyenteSitting quietly with the client
  4. Sasabihin ng nurse: 'Tell me more about what you're feeling.''Tell me more about what you're feeling.'

Sagot: B · 'Why do you feel that way?'

'Why' questions put clients on the defensive and are classed as non-therapeutic. Open-ended invitations, reflection and silence are therapeutic.
