Mga Matrix Grid at Drop-Down Rationale
Matrix Grids and Drop-Down Rationales
🔲 Isang grid na may walong findings sa gilid at tatlong column sa kabila: 'consistent with sepsis', 'consistent with hemorrhage', 'consistent with both'. Mukha itong walong tanong. Walong tanong nga ito, at magandang balita iyon.
🔲 A grid with eight findings down the side and three columns across: 'consistent with sepsis', 'consistent with hemorrhage', 'consistent with both'. It looks like eight questions. It is eight questions, and that is good news.
💡 Pangunahing ideya
A matrix is a stack of independent rows; decide each one alone. In drop-down rationale items, the condition and the evidence must match each other to score.
🧠 Bakit mahalaga
The client is at highest risk for developing [condition] as evidenced by [finding].'
MATRIX items list findings, actions or orders in rows and ask you to classify each one across columns: for example, 'indicated', 'contraindicated' or 'non-essential', or 'anticipated', 'non-essential' or 'contraindicated' for orders. Each ROW is decided on its own, usually with exactly one selection per row and one point per correct row (ZERO/ONE scoring; grids that allow several checks per row use plus/minus), so treat the grid as a stack of short questions and do not let the rows influence each other. A common matrix compares two conditions and asks which findings support each or both; findings that support both do exist, so the 'both' column is not a trap. DROP-DOWN (cloze) items complete a sentence from menus, each menu scored on its own (ZERO/ONE) unless the menus are linked as cause and evidence: 'The client is at highest risk for developing [condition] as evidenced by [finding].' Choose the condition first, then the finding that best proves it; under rationale scoring the pair must both be right to earn credit, and a triad (one condition, two findings) earns a point for each correct pair, so make sure the evidence actually supports the condition you picked. In drop-down TABLES, each cell is scored independently. Read the column headers carefully before touching a row; 'contraindicated' means harmful, while 'non-essential' means it makes no difference.
'CT of the head without contrast': anticipated.
📐 A matrix asks whether each order is anticipated, non-essential or contraindicated for a client with a suspected stroke 90 minutes from onset. 'CT of the head without contrast': anticipated. 'Heparin bolus now': contraindicated, because heparin is not used to treat an acute ischemic stroke, even once a bleed is ruled out. 'Sliding-scale insulin': non-essential right now. 'NPO until swallow screen': anticipated. Row by row, the grid is just four decisions the lesson on stroke already taught.
📌 Tandaan
- Ang bawat matrix row ay sariling tanongEach matrix row is its own question
- Ang 'Both' ay totoong column, hindi bitag'Both' is a real column, not a trap
- Dapat magkatugma ang drop-down rationale pairs: kondisyon at ang finding na nagpapatunay nitoDrop-down rationale pairs must agree: condition plus the finding that proves it

In a matrix item asking whether each nursing action is 'indicated', 'contraindicated' or 'non-essential', how should the candidate proceed?
- Pagpasyahan ang bawat row ayon sa sarili nitong meritoDecide each row on its own merits
- Piliin ang parehong column sa bawat row para manatiling pare-parehoSelect the same column for every row to stay consistent
Rows are scored independently; each requires its own judgment based on the client's situation.
Sinasabi ng isang drop-down item: 'The client is most likely experiencing [1] as evidenced by [2].' Ano ang pinakamabuting estratehiya?A drop-down item states: 'The client is most likely experiencing [1] as evidenced by [2].' What is the best strategy?
- Piliin muna ang kondisyon, saka ang ebidensiyaChoose the condition first, then the evidence
- Pumili ng kahit anong finding, dahil ang kondisyon lang ang isinu-scorePick any finding, since only the condition is scored
- Piliin ang pinakamahabang option sa bawat menuChoose the longest option in each menu
- Piliin muna ang finding at huwag pansinin ang kondisyonSelect the finding first and ignore the condition
Rationale scoring requires both parts to be correct and logically linked: pick the condition, then the finding that most directly supports it.
What does the column 'non-essential' mean in a matrix of provider orders?
- Ang order ay dapat isagawa agadThe order must be carried out immediately
Non-essential actions neither help nor harm; contraindicated actions cause harm.
