Highlight आइटम और समय के साथ trend पढ़ना

Highlight Items and Reading a Trend Over Time

🖍️ ‘उन findings को highlight कीजिए जिन पर फ़ॉलो-अप चाहिए।’ नर्स का नोट दो पैराग्राफ़ लंबा है। कुछ वाक्य चारा हैं। तरकीब यह है कि हर वाक्य को एक लैब वैल्यू की तरह पढ़ें जिसकी एक सामान्य रेंज है।

🖍️ 'Highlight the findings that require follow-up.' The nurse's note is two paragraphs long. Some sentences are bait. The trick is to read every sentence as a lab value with a normal range.

💡 मुख्य बात

केवल वही highlight कीजिए जो इस मरीज़ के लिए इस क्षण असामान्य है। Trend तालिकाओं में, दिशा अकेली संख्या पर भारी पड़ती है; तेज़ बदलाव से पहुँची एक ‘सामान्य’ वैल्यू ही असली finding है।

Highlight only what is abnormal for this client at this moment. In trend tables, direction beats the single number; a 'normal' value reached by a steep change is the finding.

HIGHLIGHT items present text (a nurse's note, a history, a table) and ask you to select the words or sentences that answer a question, most often the findings that are ABNORMAL, concerning or require follow-up. Read each sentence and ask whether it is expected for this client's age, condition and stage of care; expected findings are the bait, and highlighting them costs points under plus/minus scoring. Highlight the phrase that carries the abnormality, not the whole paragraph. TREND items, which can use any of the response formats, show vital signs or labs at several times and ask which trend is most concerning: compare each value to its earlier values, not only to the normal range. A blood pressure of 110/70 is normal, but a fall from 150/90 to 110/70 in an hour in a post-op client is bleeding until proven otherwise; a heart rate climbing from 70 to 98 is compensation even though 98 is still 'normal'. Look for pairs that move in opposite directions, such as a rising heart rate with a falling blood pressure or a rising temperature with a falling white count, and for a value that was improving and has reversed. Case studies unfold, updating the record from screen to screen, so recognizing cues often depends on direction, not snapshots.

📊 A trend table: 0800 HR 78, BP 132/80; 1000 HR 96, BP 118/74; 1200 HR 118, BP 98/60. Every 1000 value is inside the normal range. A candidate reading snapshots sees nothing until noon. A candidate reading direction sees compensation at 1000 and acts two hours earlier. The exam rewards the second candidate because real units do too.

📌 याद रखें

  • अपेक्षित findings चारा हैं; असामान्यता को highlight कीजिएExpected findings are bait; highlight the abnormality
  • हर वैल्यू की तुलना उसकी अपनी पिछली वैल्यू से कीजिएCompare each value to its own earlier values
  • उलटी दिशा की चालें (HR ऊपर, BP नीचे) और पलटाव चिंताजनक trends हैंOpposite movements (HR up, BP down) and reversals are the concerning trends

✅ खुद को परखें

एक highlight आइटम पेट की सर्जरी के 1 दिन बाद के मरीज़ में उन findings को पूछता है जिन पर फ़ॉलो-अप चाहिए। उम्मीदवार को कौन सा वाक्यांश highlight नहीं करना चाहिए?A highlight item asks for findings that require follow-up in a client 1 day after abdominal surgery. Which phrase should the candidate NOT highlight?
  1. ‘पेट फूलने के साथ bowel sounds अनुपस्थित’'Absent bowel sounds with abdominal distention'
  2. ‘Room air पर oxygen saturation 90%’'Oxygen saturation 90% on room air'
  3. ‘दवा के बाद चीरे का दर्द 3/10’'Incisional pain 3/10 after medication'
  4. ‘Temperature 38.9 °C’'Temperature 38.9 °C'

उत्तर: C · ‘दवा के बाद चीरे का दर्द 3/10’

नियंत्रित चीरे का दर्द ऑपरेशन के बाद की अपेक्षित finding है; बाकी असामान्य हैं और उन पर फ़ॉलो-अप चाहिए।

Controlled incisional pain is an expected post-operative finding; the others are abnormal and need follow-up.

एक trend तालिका दिखाती है कि ऑपरेशन के बाद के मरीज़ की heart rate तीन घंटे में 72 से बढ़कर 98 हो गई जबकि blood pressure 128/82 से गिरकर 108/68 हो गया। सभी वैल्यू सामान्य सीमा के भीतर हैं। उम्मीदवार को इसकी व्याख्या कैसे करनी चाहिए?A trend table shows a post-operative client's heart rate rising from 72 to 98 over three hours while blood pressure falls from 128/82 to 108/68. All values are within normal limits. How should the candidate interpret this?
  1. दर्द की दवा के प्रति एक अपेक्षित प्रतिक्रियाAn expected response to pain medication
  2. कोई चिंता नहीं, क्योंकि हर वैल्यू सामान्य सीमा के भीतर हैNo concern, because every value is within normal limits
  3. मरीज़ की स्थिति में सुधारImprovement in the client's condition
  4. एक चिंताजनक trend जो शुरुआती hypovolemia का संकेत देता हैA concerning trend suggesting early hypovolemia

उत्तर: D · एक चिंताजनक trend जो शुरुआती hypovolemia का संकेत देता है

Heart rate और blood pressure की उलटी चाल volume की कमी या रक्तस्राव के लिए compensation का संकेत है; दिशा सामान्य रेंज से ज़्यादा मायने रखती है।

Opposite movement of heart rate and blood pressure signals compensation for volume loss or bleeding; the direction matters more than the normal ranges.

Highlight आइटम पर plus/minus स्कोरिंग में, किसी अपेक्षित, सामान्य finding को highlight करने का क्या नतीजा होता है?Under plus/minus scoring on a highlight item, what is the consequence of highlighting an expected, normal finding?
  1. उम्मीदवार को दोबारा शुरू करना पड़ता हैThe candidate must restart
  2. स्कोर पर कोई असर नहींNo effect on the score
  3. एक अंक घटा दिया जाता हैA point is subtracted
  4. आइटम अपने आप सही हो जाता हैThe item is automatically correct

उत्तर: C · एक अंक घटा दिया जाता है

गलत highlight अंक घटाते हैं, इसलिए चुनिंदा होना मायने रखता है।

Incorrect highlights subtract points, so selectivity matters.
