The Diet Orders You Must Not Mix Up
🍽️ A client with kidney failure is handed a banana and orange juice 'because potassium is healthy'. A client with celiac disease gets oatmeal. Both trays are wrong, and both are classic exam items.
💡 Key idea
Renal restricts potassium, phosphorus, sodium and fluid. Celiac bans wheat, barley and rye. Dumping syndrome wants small, dry, protein-rich meals and lying down afterward.
🧠 Why it matters
CLEAR LIQUID: broth, gelatin, clear juice, tea; FULL LIQUID adds milk, cream soups, ice cream; MECHANICAL SOFT and PUREED for chewing or swallowing difficulty. CARDIAC and hypertension: sodium about 2 g a day and low saturated fat; teach clients to read labels and avoid canned soups, deli meats and pickles. RENAL (dialysis): restrict potassium, phosphorus, sodium and fluids, with protein adjusted to the stage; avoid bananas, oranges, potatoes, tomatoes, dairy and cola. CELIAC: gluten-free, so no wheat, barley or rye, and only certified oats; rice and corn are fine. DUMPING SYNDROME after gastric surgery: small frequent meals, high protein and fat, low simple carbohydrate, no liquids with meals, and lie down for 20 to 30 minutes after eating. GOUT: low purine, so limit organ meats, shellfish and alcohol. HIGH FIBER for constipation and diverticulosis; LOW RESIDUE during a diverticulitis flare. CLIENTS on WARFARIN keep vitamin K intake consistent rather than eliminating it. DIABETES: consistent carbohydrate counting, not 'no sugar'.
🏥 In the real world
🥣 After gastric bypass a client reports cramping, sweating, dizziness and diarrhea 20 minutes after every meal. The nurse's teaching: eat six small meals, skip the juice and the sweet rolls, add protein, drink between meals rather than with them, and rest lying down afterward. The symptoms stop. No medication was involved, just the right diet order.
📌 Takeaways
- Renal: low potassium, phosphorus, sodium, fluids
- Celiac: no wheat, barley, rye; dumping: small frequent meals, no liquids with meals
- Warfarin: consistent vitamin K, not zero
📖 Terms in this lesson
Dumping syndrome: Cramping, sweating and diarrhea after meals when food rushes from the stomach into the intestine after gastric surgery.
✅ Test yourself
Which food should the nurse remove from the tray of a client with end-stage kidney disease on hemodialysis?
- Steamed white rice with unsalted butter
- Peeled apple slices with cinnamon
- Green beans with black pepper
- Baked potato with cheese
Answer: D · Baked potato with cheese
Potatoes are high in potassium and cheese is high in phosphorus; both are restricted in renal diets.
Which meal is appropriate for a client with celiac disease?
- Pasta with marinara sauce and parmesan
- Grilled chicken with rice
- Whole wheat toast with jam
- Barley soup with rye crackers
Answer: B · Grilled chicken with rice
Rice and plain protein and vegetables are gluten-free. Wheat, barley and rye products contain gluten.
A client with dumping syndrome asks how to prevent symptoms. Which teaching is correct?
- Eat small, frequent, high-protein meals
- Choose high-sugar foods for quick energy
- Drink a large glass of water with each meal
- Eat three large meals daily
Answer: A · Eat small, frequent, high-protein meals
Small, protein-rich, low-simple-carbohydrate meals without liquids slow gastric emptying; resting afterward reduces symptoms.
